The Life of an HCA Nurse
The Oncology Nurse Liaison functions as a member of the multidisciplinary team, to ensure care of the patient is streamlined through the cancer journey while keeping the PCP abreast of what is happening with their patient. The Oncology Nurse Liaison’s primary function is to build and foster relationships between the cancer team and the PCP, helping to facilitate the PCP remain primary in all care needs.
Responsibilities: * Build collaborative relationships with referring physician/providers for oncology care. * Track and document interventions and outcomes. * Serve as patient advocate as directed by Primary Care Physician * Respond to patient, family, and provider challenges until resolution is achieved. * Provide appropriate teaching, outreach, and education to patients and families. The aim of this work being to ensure the patient is empowered to manage his or her own health. * Drive process improvement. * Stay current on the latest oncology nursing developments * Ensure service continuity. *
- Graduation from an accredited School of Nursing - Bachelors degree required
- Current Missouri or Multistate RN license required.
- Five years oncology nursing experience required.
- Experience in program development highly preferred.
- Experience as a nurse navigator and / or patient educator in oncology preferred.
