Role Purpose:

The clinical pharmacist works as part of the interdisciplinary care team to provide optimal clinical services to patients across the continuum of care. The clinical pharmacist facilitates the development, implementation and maintenance of systems that promote desirable patient outcomes through the use of safe and cost-effective drug therapies. Daily responsibilities include thorough medication reconciliation, individualized patient medication education, disease state pharmacotherapy education, medication profile evaluation as well as coordinating additional clinical pharmacy services. The clinical pharmacist has the opportunity to actively participate in a variety of medication related projects as the pharmacy practice model evolves. Collaboration with other health care professionals occurs via participation in hospital committees and service as a drug information specialist for physicians, nurses and other health professionals.

Responsibilities:

  • Age Specific Criteria: Demonstrates and maintains knowledge and skills necessary to care for the patient group of: 1) Infant, 2) Pediatric, 3) Adolescent, 4) Adult and 5) Geriatric.

  • Interprets and clinically evaluates medications orders for correct indication, dosage, contraindications, interactions and allergies.

  • Ensures the daily drug dispensing process is completed efficiently and accurately.

  • Facilitates the identification, resolution, implementation, and communication of cost-effective medication use issues in collaboration with the medical staff.

  • Assists in the development and implementation of treatment guidelines, protocols, formulary changes, and critical medication pathways.

  • Provides clinical consultation and clarification to medical staff. Suggests appropriate, cost-effective therapeutic alternatives to medical staff as needed.

  • Provides accurate, adequate, and timely drug information, in oral or written form to the professional staff. Maintains the material necessary to provide this information.

  • Provides additional clinical interventions and reviews as requested.

  • Reads and evaluates the medical literature for studies supporting the pharmaceutical care and pharmacoeconomic programs.

  • Contributes to the advancement of the profession through presentations at local, state, and national meetings.

  • Acts as an integral part of the pharmacy team, providing leadership and training to other staff members when necessary.

Qualifications

Minimum Qualifications:

  • Graduate of accredited school of Pharmacy, Pharm D or M.S. ASHP accredited residency and/or 2-4 years Clinical Pharmacist experience preferred but not required.

  • Current Missouri State license as a Pharmacist or eligible for Missouri licensure within 90 days of start.

  • 2-4 years Clinical Pharmacist experience and/or ASHP accredited residency preferred but not required.

  • Time management skills and ability to prioritize workload. Demonstrate computer proficiency and knowledge of pharmacy automation dispensing systems. Demonstrate attention to detail. Demonstrate excellent patient care skills. Demonstrate ability to work independently. Demonstrate strong leadership skills and ability to work as part of an interdisciplinary team. Demonstrates excellent written, verbal, and interpersonal skills.

  • Stooping, kneeling and/or crouching daily.

SSM Health St. Mary's Hospital – Jefferson City –

SSM Health St. Mary's Hospital - Jefferson City is a 167-bed, full-service community hospital located in Missouri's state capital. The hospital moved to a new, state-of-the-art building in November 2014. The hospital features a dedicated inpatient pediatric unit, a Wound Center and Outpatient Treatment Center. Other services offered at the hospital include emergency services, cardiology, orthopedic care, cancer care, outpatient surgical services, maternity care and women's health.
